The following citizens (residents of Macau) are exempted from payment:20h00
Pregnant, parturient, puerperal patients (until the end of the first month after birth),(present the identity document); Children until 10 years-old (present the identity document); Students of primary and secondary schools (present the Student Card issued by the Department of Education and Youth); Indigents (present a valid Certificate of Indigence issued by the Macau Social Welfare Department); Persons over 65 years-old (present the identity document); Cancerous patients, the mentally-handicapped, carriers of infectious-contagious diseases and drug-addicts (present the respective Access Card to Health Care, which is issued by presentation of Medical Certificate); • Access Card to Health Care holders, those card must be issued by Social Work Service of the Department of Health, Macau SAR; • Users who are Macau Resident Identity Card holders and have access to the Health Centres. Documents to be submitted: The computerized card issued by the DH. In case it is necessary also submit: Access Card to Health Care for civil servants and their family members who receive family subsidy. Medical Assitance Card for workers who are working in services that have signed a protocol with the Health Department. Users who are exempted of payment should present the document certifying that exemption (See "payment exemption"). Contact: The Macau Public Health Laboratory (PHL), performs all actions required to learn about risk factors concerning health, the epidemiological situation and the most relevant diseases affecting the community. Services provided: 1) Laboratorial tests in the areas of Environmental Health (Water and Food) and the most relevant Disease-Carriers in the Region; The requests for assistance in the area of Environmental Health, except those which were previously agreed with other entities, should be addressed to the Director of the Health Department. The requests for assistance in the ambit of Disease-Carriers should be addressed through the clinical services of the DH. 2) For obtention of information concerning infection by HIV dial the following telephone numbers: 5040103/5040105. Timetable for delivery of samples: The Blood Transfusion Centre is a division of the DH that was created in 1988, being its main task to assure blood self-sufficiency in the Region. All blood donors are voluntary and good-will. The blood is collected and submitted to rigorous analysis and distributed gratuitously to official and private Health Establishments. The citizens of Macau when becoming blood donors are contributing to save lives or to mitigate the suffering of numerous sick people. Remarks: If the applicant is working in the healthcare industry and has registered in the Department of Health, items 4 and 5 can be omitted.
Renewal Procedures and Necessary Documents 1) The licence is valid for one year. Renewal must be made within 60 days after the expiry date. Otherwise, the licence will become invalid. 2) Copy of licence or the renewal certificate of previous year
Approval Duration In general, it takes 65 days to process the application. However, if the applicant cannot meet the requirements of licence issuance or the facilities and equipment in the premises cannot meet the hygienic regulations, the above-mentioned period will be extended accordingly.

Target Those who are pursuing the professions of physicians, dentists, doctors of Traditional Chinese Medicine, odontologists, Chinese herbalists, masseurs, acupuncturists, therapists, medical assistant technicians and nurses must apply for licences.
Application Procedures and Necessary Documents The applicant should visit Medical Activity Licensing Unit in person and submit the following documents: 1) a licence application letter to Director of Department of Health 2) Declaration of Absence of Conflicts which declares the applicant does not have any business in conflict with the licence application 3) Copy of the Macao SAR Resident Identity Card 4) Certificate of Criminal Record (This certificate can be applied in person at Identification Department. For certificate collection, the applicant has the option of collecting it in person at Identification Department or via the assistance of Department of Health.) 5) Certificate of Physical and Mental Health (Macao Civil Servant Statute Annex Format 1) (Application at Health Centre) 6) Certified copies of relevant academic certificates, diplomas and transcripts 7) Copy of course description
Remarks: 1) Chinese herbalists need to submit the Certificate of Recognition issued by the Chinese Herbalists Qualification Recognition Assessment Committee which is operated within the scope of the Department of Health. 2) All documents will be processed and assessed by Private Medical Activity Licence Technical Committee. 3) After passing the assessment by Private Medical Activity Licence Technical Committee, the applicant needs to submit the floor plan of the clinic, as well as a list of equipment to be installed in it, an advertisement /sign board layout and advertisement application form, the inspection report issued by Property Registration Department or the certificate of use permit issued by Lands, Public Works & Transport Bureau within 60 days. 4) After passing an on-site hygiene inspection of the venue, an approval will be dispatched by the Deputy Director of General Health Care Sub-Board, which will be published in the ?acao SAR Gazette?

Target Private healthcare unit that is equipped with in-patient department and post-operative resuscitation room.
Application Procedures and Necessary Documents Application should be submitted with the following documents: 1) Application letter to the Director of Department of Health and the content should include the following: a) Company name or name of natural person, and other identification information of the applicant; b) Address of legal entity or the residential address of natural person; c) Name and location of the unit; d) Information about the medical management members e) Types of services to be provided.
2) Supplementary documents that should be attached to the application letter: a) Certified copy of identification document of the applicant, for the case of legal entity, certified copies of establishment documents and Articles of Association or a copy of the Macao SAR Gazette where the establishment documents and Articles of Association were published, and certified copies of the identification documents of the management team; b) The latest Certificate of Business Registration; (this is not applicable for charity organizations) c) Certificates of Criminal Record of the applicant or the management team members of the legal entity; d) A detailed description of staff members and staff list; classified under the following 4 categories:
e) A structural plan of the building that is complied with the current local regulations, together with the use permit issued by Lands, Public Works and Transport Bureau; f) Functional layout and specification memorandum of the facilities, together with the layout plan showing the operation of the facilities in the healthcare unit, which was signed by a qualified technician; (including the functional layout, specification memorandum and floor plans of each floor in each block of each hospital building) g) Draft of internal regulations; h) Declaration of agreement to perform the technical functions in the unit by designated technical instructors (including medical superintendent, hospital administration departments, nursing departments and pharmaceutical affairs departments).
Fees 1. The fee for the operation permit of private healthcare unit is MOP4,000.00. Annual renewal is required with a fee of MOP400.00. 2. The fee for the licence of the unit is MOP5,000.00. Annual renewal is required with a fee of MOP500.00.

In other cases, by certificate of recognition of the qualifications issued by the Department of Higher Education or by the Macau Health Department according to whether there are academic or professional qualifications, respectively; 3) Medical certificate, issued by the medical authority, certifying that the interested person has health, physical and psychic conditions to exercize the profession (the interested person should go to the Health Centre nearest to its residence to obtain such) 4) Certificate of residence in the Region; 5) Declaration signed by the interested party, stating that he/she does not have any incopatiblity to exercize the profession; 6) Certificate of no criminal record; 7) Copy of the identity document.
